Current Rate of Pay: Min $42.91- Max  $56.27

Current Shifts:  Mon-Fri, 8 hour shifts

Position Summary:

The Diabetes Patient Educator implements a variety of Diabetes education programs for in-patients and ambulatory care patients at Guelph General Hospital and is responsible for assessing needs and delivering direct diabetes education and clinical support to both individuals and groups.

The Diabetes Patient Educator provides high quality, safe, effective and evidence-based care in partnership with patients and families, in accordance with the College of Nurses of Ontario standards and GGH’s mission, vision, values and policies.  The Diabetes Patient Educator optimizes the patient’s experience and care outcomes by:

  • Being accountable for one’s decisions and professional practice;
  • Establishing a therapeutic relationship with the patient and family;
  • Collaborating with other members of the patient’s care team;
  • Coordinating, communicating and documenting patient care activities.

Qualifications, Experience, Skills and Abilities:

  • Certificate of registration as a Registered Nurse with the College of Nurses of Ontario
  • Current Certified Diabetes Educator (CD) designation through the Canadian Diabetes Certification Board (CDEB) preferred
  • Baccalaureate Degree (preferably in Nursing)
  • BLS certification
  • Experience providing effective, evidence based clinical education
  • Demonstrated commitment to best practice, nursing standards and life-long learning
  • Demonstrated knowledge of the RNAO Best Practice Guidelines
  • Demonstrated knowledge and experience in child and adult education principles
  • Demonstrated leadership and facilitation skills
  • Excellent interpersonal and communication skills, both verbal and written
  • Strong organizational and time management skills
  • Strong critical thinking, problem solving and decision-making skills
  • Demonstrated ability to work in a collaborative practice model
  • Current knowledge and experience with quality improvement processes
  • Demonstrated ability to work independently as well as collaboratively within a team in a fast-paced and ever-changing environment
  • Demonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physicians
  • Ability to use related computer programs (Meditech, Windows, MS Work, Excel, PowerPoint) and familiarity with electronic documentation systems
